In order to ensure timely university registration in Italy, please include the following with your application:

• Notrarized, apostilled, enrollment verification from your registrar.  Please give one of the following samples to your university registrar to use:
• Ferrara sample enrollment verification
• Florence sample enrollment verification
  
  Ask your registrar to write the letter and have his/her signature notarized, then send the notarized letter to the Secretary of State's office in your state for an apostille.  Include an envelope so the Secretary of State can forward the aposilled letter to our office address:

• 3 typed copies of the Attestato (type directly on this form, print three copies, sign all three)--we have translated this form into English to help you complete it.
• 3 copies of the Al Magnifico Rettore (original signatures on all 3--this form may be handwritten)

• Florence Al Magnifico Rettore
• One Informazione Personale (this form may be handwritten)
• Copy of the ID page and facing page (the page with the photo and the page with your signature) of your passport
• 4 photos with signatures along the right-hand edge, signed bottom to top--see example.  You will later be asked for 4 additional photos for our office—please have them all taken at once, as duplicates are less expensive than originals.
