REQUIRED PAPERWORK AND INFORMATION BY UNIVERSITY

In addition to applying to Middlebury College, your host university requires you to complete some paperwork. Though not an application, per se, these are the necessary items to complete your file. This is important to do now, at the time of application, in order to get you registered at the university—this is what starts the visa process for those of you who need visas too.

DUE DATES

These materials are due by March 1 for students going abroad in the fall and by October 1 for students going abroad in the spring.  Please submit them with your Middlebury application materials.

ARGENTINA
Universidad de Buenos Aires:

  • Preliminary list of courses to be taken.  Although you will only take two or three courses at the university, please list five per semester. Remember all courses must be within the same facultad: filosofia y letras or sciencias sociales.
  • Enrollment verification indicating you are a full-time student and your intended date of graduation.  Midd students: you may print this yourself from BannerWeb; Students from other colleges and universities: check to see who on your campus processes these, though it is often the Registrar's office and many schools have it on-line, which is fine.
  • Passport Photocopy.

Universidad del Salvador:

  • Completed university application forms. Find courses here
  • Passport photocopy.
  • Cover letter: essay in Spanish about why you want to attend USAL (1-2 paragraphs is fine).
  • 2 academic letters of recommendation.  Midd students should request these from two professors from any discipline--there is no form--just ask for a letter (English is fine); I will copy the recommendations I already have for students from other universities--no need to request more.
  • Medical Certification (letter from the doctor certifying that you are in good health).
  • Photocopy of your health insurance card, front and back.
  • Proof of medical insurance that includes repatriation and medical evacuation--a copy of your policy if repatriation and medical evacuation are included, OR proof (receipt) that you bought the International Student ID Card, which also covers these two items


Universidad DiTella:

  • Completed university application forms.
  • Passport Photocopy
  • Cover letter: essay in Spanish about why you want to attend DiTella (1-2 paragraphs is fine)
  • 2 passport size photographs (you will later be asked for 2 more and you should take extras with you--I would go ahead and get 6-8 now, as duplicates are less expensive than originals).
  • Photocopy of your health insurance card, front and back.
  • Proof of medical insurance that includes repatriation and medical evacuation--a copy of your policy if repatriation and medical evacuation are included, OR proof (receipt) that you bought the International Student ID Card, which also covers these two items.

Tucumán:

  • Preliminary list of courses to be taken--begin here ; click “carreras”; you will be given a list of facultades under which you will find active links for the different majors offered in each of the departments; click a major to get to the home page of the facultad, then click the major again on the fac’s homepage to see the courses offered (on the right).
  • Enrollment verification indicating you are a full-time student and your intended date of graduation.  Midd students: you may print this yourself from BannerWeb; Students from other colleges and universities: check to see who on your campus processes these, though it is often the Registrar's office and many schools have it on-line, which is fine.
  • Passport Photocopy.

BRAZIL
Belo Horizonte:

  • Completed university application forms.
  • Financial resources declaration (third page of the above application).
  • Certificate of previous studies (English or Portuguese)—I will assemble a list of your previous Portuguese courses.
  • Cover letter: essay in Portuguese about why you want to attend PUC Minas (1-2 paragraphs is fine).
  • Brazilian Consulate in the U.S. through which you will apply to obtain your visa.
  • Passport Photocopy.

Florianópolis:

  • Completed university application.
  • Enrollment verification indicating you are a full-time student and your intended date of graduation.  Midd students: you may print this yourself from BannerWeb; Students from other colleges and universities: check to see who on your campus processes these, though it is often the Registrar's office and many schools have it on-line, which is fine.
  • Preliminary list of courses to be taken--please list five per semester.
  • Brazilian Consulate in the U.S. through which you will apply to obtain your visa.
  • Passport Photocopy.

Niterói:

  • UFF application: please return as an attachment to me and I will email everyone's together.
  • University housing form: please return as an attachment to me and I will email everyone's together.
  • Cover letter: essay in Portuguese about why you want to attend UFF.
  • Preliminary list of courses to be taken (select graduação, then cursos oferecidos; select a subject, then licenciatura)--please list five per semester.  *NB: It is best to take courses within one facultad, as the scheduling will be easier and you’ll have exposure to the same group of students, but under no circumstance should you select courses from more than two different facultades.
  • Brazilian Consulate in the U.S. through which you will apply to obtain your visa.
  • Passport Photocopy.

CHILE
(All universities):

  • Consulate through which you will apply for your visa.
  • Preliminary list of courses to be taken--please list five per semester. Follow these instructions for navigating the universities’ websites to find courses. 
  • Enrollment verification indicating you are a full-time student and your intended date of graduation.  Midd students: you may print this yourself from BannerWeb; Students from other colleges and universities: check to see who on your campus processes these, though it is often the Registrar's office and many schools have it on-line, which is fine.
  • Passport Photocopy
  • 2 Passport photos (you will later need to return others with your pre-departure materials, and it’s not a bad idea to take some with you. As duplicates are less expensive than originals, I’d recommend getting 8 now to save you time and money later).
  • For Universidad Austral ONLY: university application form.  You do not need to include the list of materials at the end (many are asked for above on this list), but you will need to take #7-10 abroad with you. 

MEXICO
Guadalajara :

  • Completed university application forms.
  • Cover letter: essay in Spanish about why you want to attend Un. de Guadalajara (1-2 paragraphs is fine).
  • Spanish language reference (I will copy the one from your file).
  • Recommendation letter in English or Spanish (Midd students: please ask for one from a professor who knows you well--someone other than the professor who completed your language assessment; students from other universities: I will copy the one I already have).
  • Housing Form (this is our own form, but the university assists us in placing students, so please complete it now). It requires that you attach one photo.
  • Internship application (if applicable): cover letter describing the type of internship you'd prefer and resume in Spanish.
  • Passport Photocopy.
  • Consulate through which you will apply for your visa.
  • Photocopy of your student ID
  • Photocopy of your health insurance card—front and back.
  • 3 additional passport photos (not including the one on the housing form).  Go ahead and get at least 8, as you will need them for your pre-departure requirements as well as your visa.

Xalapa:

  • Completed university application forms. Click “Admissions” on the left, then click “1. Application form.” (Everything else you need is listed below—you must do the placement test, but DO NOT send the fee.) To actually choose your courses, DO NOT select them from this section for international students--choose from the universities offerings: select Oferta Academica from the left column, then select Sistema Escolarizado, select a field of study from the middle of the page (normally humanidades). You'll see a chart and if there is an X in the column after the course title, it is available in Xalapa. Other courses may be found by scrolling to the bottom of the page and selecting a specific major.
  • Cover letter: essay in Spanish about why you want to attend Un. de Vercruzana (1-2 paragraphs is fine).
  • Recommendation letter in English or Spanish (Midd students: please ask for one from a professor who knows you well--someone other than the professor who completed your language assessment; students from other universities: I will copy the one I already have--no need to request another).
  • Housing Form (this is our own form, but the university assists us in placing students, so please complete it now). It requires that you attach one photo.
  • 3 additional passport photos (not including the one on the housing form).  Go ahead and get at least 8, as you will need them for your pre-departure requirements as well as your visa.
  • Certificate of good health—a letter from your doctor on letterhead that states you are in good health.
  • Letter from your financially responsible party (parent) stating that s/he assume full financial responsibility for you during the period (give months and year) of your study abroad program.
