Faculty members with contracts beginning September 1, 2009 will receive their benefits package by mid-July. This package contains detailed information on Middlebury College's benefit plan, enrollment forms, and deadline information.

Please note:
- You are enrolling in benefits for the period September 1, 2009 through December 31, 2009. (In the fall HR will be conducting an open enrollment for benefits effective January 1, 2010.)
- If your employment documentation (I-9, W-4, & Visa (if required) is not received by September 1, 2009, then your benefits will begin on the 1st day of the month following their completion.)
- If you do not submit the enrollment forms by the deadline you will not be able to enroll in the medical, dental, flexible spending accounts, or supplemental Life/AD&D benefits until January 2010.

Retirement Plans Information & Links
Two retirement plans are available, the Core Plan (401a),
which includes both employee contributions and a generous
employer contribution, and the Voluntary Plan (403b), for
those who wish to make additional retirement savings.

New employees may qualify to go directly to the 9% or 15%
employer contribution level if the employee had participated
in a 401a, 401k, or 403b plan with a previous employer
within 6 mounths of the date of hire with Middlebury AND
that previous employer contributed in excess of 3% to the
retirement plan. Various restrictions apply: See HR for details.

Voluntary Retirement Plan Enrollment Form
Domestic & Civil Union Partnerships Domestic partners (if registered with the Human
Resources office) and civil union partners, are eligible
for all spousal-type benefits under the Plan except the
Health FSA and the Dependent Care FSA.
Domestic partners generally must be enrolled for coverage
at the time you, the employee, are first eligible for coverage
under the Plan or during any subsequent enrollment period.
Your domestic partner will not be allowed to enroll for
coverage during the Plan Year unless he or she has been
identified as your domestic partner in the records of Human
Resources for at least six months, and he or she has lost
coverage under another benefit plan.
