This form should be used for all employees who supervise other College employees.
The form is divided into eight parts:
General – the housekeeping part of the form - were evaluations and timesheets handled properly?
Employee development – is the supervisor working to develop their employees? Note: the last question that you will see in several places on the form, asks how you obtained the information to answer the questions. Did you speak to the supervised employee(s), did you check their files, did you speak with the supervisor, did you observe the behavior?
Communication – is communication effective?
Goal Setting and Planning – are goals set, explained, followed? Does employee understand the mission of the department and how the goals and expectations fit?
Employee relations – what is the climate within the department or the work crew? These questions are designed to find out how the working relationship is maintained.
Team Building and/or coaching efforts - further defines the climate, tries to determine the level at which the work group is functioning as a team or effectively as individuals.
Safety – this is part of our continuing effort to be sure that not only employees but supervisors understand their responsibility as it relates to safety.
Budget questions – this needs to be completed for those employees who are budget administrators. Here we are trying to determine the level at which they operate as budget administrators, how they perceive their job as a budget administrator.
