What you need to fill out your state income tax return:
- Your Federal Income Tax Return for that year.
- Social Security Number (SSN) or Individual Taxpayer Identification Number (ITIN) if applicable
- Form W-2 (employment income) if any for that year (if you do not have it anymore, go to BannerWeb to print it out) and/or contact your former employer(s)
- Form 1042-S (employment income or scholarship income) if any for that year (if you do not have it anymore, email the Taxmanager for a copy) and / or contact other places where you worked and / or received financial aid
- Go to the website of the Vermont Department of Taxes, select the year for which you need the tax form and click go.
- Choose the Income Tax Return Booklet.
- Within the booklet you will need Form IN-111 if you only earned monies in Vermont that year or Form IN-113 if you earned monies in other states as well. In addition, you will need the taxtable at the end of the booklet.
How to fill out the tax return:
- Under Taxpayer Information enter your First and Last Name, your Middlebury address and your SSN or ITIN.
- The Vermont School District Code is 120.
- Under Tax Filing Information you most likely file as Single and with 1 exemption. Enter the rest of the information by going to your Federal Income Tax Return for that year to the appropriate box.
- For the Federal Return Information follow again the instructions.
- Use the final number of the Vermont Taxable Income to go to the Tax Table at the end of the booklet and find your amount of taxes you owe the State of Vermont.
- Under Number 7 Payments and Credits enter the amount of Vermont tax that has been withheld so far (from your W-2).
- The difference between 5) and 6) above will be the amount that you owe to the State of Vermont or will receive a refund for.
- When you are done, print the form, sign and date it. Attach a copy of each W-2 and 1042-S to it.
- Make a copy of the entire package for your records.
- Keep it in your records for at least 3 years.
- Include a check if you owe monies.
- Send to the address as provided in the booklet.
