Vision Insurance

VSP (Vision Service Plan)

Middlebury College's vision insurance covers eye exams, glasses, and contact lenses.  You can choose to see the eyecare provider who's right for you—a VSP doctor or any other provider.  No ID card is required (just tell your VSP provider the last 4 digits of your SSN and date of birth).

You have a choice between two vision plan options, both provided by Vision Service Plan (VSP). There is a Base Plan and an Enhanced Plan with additional benefits.

If you choose to see a provider other than a VSP doctor, your benefit may differ from the coverage you receive with a VSP doctor. You also will not receive the eye health benefits your plan offers. VSP doctors can detect signs of health conditions and may assist you in coordinating care with your primary care physician, if necessary, to keep you and your eyes healthy.

To request a reimbursement after you have seen a out-of-network provider please fill out the Member Reimbursement Form or go onto VSP's website ( and click on Member Reibursement Form under My Benefits on the left hand side of the page (after you have logged in).  Follow the instructions on the page to submit your reimbusement form and itemized receipt.  Please allow up to 10 business days (plus mailing time to and from VSP) for VSP to process your reimbursement.
