Staff Resignation Form

**TO BE COMPLETED BY SUPERVISORS**

As soon as you are notified that one of your employees will leave employment at Middlebury, please complete the following form to begin the formal resignation process.

Enter the name of the employee who will be leaving Middlebury.

Enter the Middlebury ID number of the employee who will be leaving the College.

Enter the email address of the employee who will be leaving their employment at the College.

Enter the last day the employee will be physically present at work. (The last day can't be a CTO day or a holiday.)

Select the appropriate type of employment change based on the information below.

Did the employee submit a letter of resignation?

Select whether the employee is in a benefits eligible position.

If this employee is a supervisor, please enter the name of the employee who will assume their time approver responsibility.

Describe the employee's reason for leaving their employment at Middlebury.

If you plan to replace this position you must submit a requisition in Greentree. Look for instructions under "Greentree Help" in the sub-menu in the left hand column below "Managers & Supervisors".
