Winter Term Application for Internship Credit

Please note that students are required to show you a copy of their personal statement/cover letter and internship description for your review before you complete this form.

Please also note that you may have been asked to complete this form as a Faculty Adviser, Academic Sponsor, or both. Please only complete the parts of the form (only A, only B, or A and B) that pertain to this relationship.

A. Faculty Adviser Approval
Review

I have reviewed the internship description, read the student’s personal statement, and discussed the internship with my advisee. I believe this internship will benefit him/her and help achieve the stated academic objectives. I, therefore, approve it.

B. Academic Sponsor Approval

The role of the Academic Sponsor is to provide guidance to the student before, during and after the internship is completed. After discussing the experience and on the basis of the student's written work, the Academic Sponsor will submit a recommendation of "credit" or "no credit" to the Career Services Office
