Incident Reporting

A safety incident (accident, fire, spill, animal bite, etc.) is an event that results in an injury or illness to an individual, damage to equipment/building, or a “near miss”.  It is important to follow proper reporting procedures after a safety incident. 

Complete the Middlebury Sciences Incident Report  for all safety incidents.

If a safety incident involved an injury—

Faculty and staff must report all injuries to manager/supervisor immediately.  Report to Human Resources within 24 hours of the incident.

Students must report all injuries to your supervisor or lab instructor immediately.

In the event of accident or medical emergency, call the Department of Public Safety at x5911 (802-443-5911) or dial 911
