Incident Reporting
A safety incident (accident, fire, spill, animal bite, etc.) is an event that results in an injury or illness to an individual, damage to equipment/building, or a “near miss”. It is important to follow proper reporting procedures after a safety incident.
Complete the Middlebury Sciences Incident Report for all safety incidents.
If a safety incident involved an injury—
Faculty and staff must report all injuries to manager/supervisor immediately. Report to Human Resources within 24 hours of the incident. http://www.middlebury.edu/offices/business/hr/staffandfaculty/safety
