  • Transfer Credit Form
  • Use this form to apply for pre-approval of courses to take off campus, or for approval of major or other college requirements for courses already completed.
  • Major/Advisor Declaration or Change Form
    Required for major types: single, double and joint Environmental Studies. Download this form, have it signed by your new advisor and the chair/director and turn it in to the Registrar’s Office before the end of your second full year of studies.
  • Joint Major Declaration Form
    Required for all joint majors except Environmental Studies. Download this form, have it signed by your new advisors and turn it in to the Registrar’s Office before the end of your second full year of studies.
  • Minor Declaration Form
    Minors must be declared by the end of the Add Period of the student’s seventh semester.
  • Degree Audit
    Sample form seniors may use to record degree progress in consultation with advisor and department chair/program director in the first semester of the senior year. 
  • Special Student Application
    Seniors who have only one or two credits to complete and plan to live off campus can apply for this part-time status in their last semester.
  • Authorization to Send Grades
    Students may fill out this form each semester to have grades sent directly to others.
  • Request for Cross-Listed Subject (login required)
    Students may fill out this form to request a cross-listed subject appear on the transcript.
  • Request to Register for 4.5 Credits
    NOTE: This form requires an upload of advisor’s approval, so be sure you have that before you begin the form. Once you submit the form, the Office of the Registrar will add you to the class. Please use your Middlebury credentials to login via Google

  • Change of Grade Form
  • Email the completed form to the Administration Committee, care of Angela Rheaume  (admincommittee@middlebury.edu). 
  • D & F Form (login required)
    Submit this form if you have assigned a final grade of D or F to a student. Please use your Middlebury credentials to login via Google.
  • To resolve an Incomplete grade for a Vermont undergraduate student, Faculty can find the the Incomplete Resolution Form in their Banner Web
  • Schools Abroad D & F Form
    Directors submit this form if a grade of D or a failing grade has been assigned to a student. Please use your Middlebury credentials to login via Google.

VA Enrollment Certification Requests

Middlebury partners with the VA in support of educational benefits programs including the Post-9/11 GI Bill® and the Yellow Ribbon program.

NOTE: According to VA regulations, educational benefits may be applied only to Middlebury programs offered at our Middlebury, Bennington, Oxford, Paris, Amman, Yaroslavl, Moscow, Madrid, Montevideo, Buenos Aires, and Florence locations.

Benefits eligible students should complete the following steps.

Step 1:  Submit the Request for Certification form

Step 2: Chapter 33 students who are 100% benefits eligible should also submit the Yellow Ribbon Request form.

Step 3: Submit a copy of your VA Certificate of Eligibility (COE) to:

Step 4: Guest students, students who are pursuing a degree at another institution and plan to apply these courses toward their degree at their home institution, must submit a primary school letter prepared by the home institution to the School Certifying Official (see above). The primary school letter must grant approval for the Middlebury courses to count toward the degree program and must include the specific Middlebury course names and numbers.
