Primary Computer Upgrade Request Form

If you have inquired about when your primary computer is due for upgrade and did not receive any infomation, please answer these questions so we can schedule your upgrade during an upcoming computer upgrade cycle. Thanks!

Enter your username/account name to access the Middlebury network.

You will need to find the Middlebury Asset Tag number. This number is located on a small blue & silver metal tag on or under your computer. It is a number from 4-6 digits in length.

Select whether you have a laptop or desktop computer.

Please indicate whether you have an Apple (Mac) computer or a Windows based PC.

Optional: To assist us better you can send along the service tag or serial number of your current primary computer. This information can be found on a vendor tag or etched into the computer.
