Summer program students, faculty, staff, visitors, and associated family members coming to campus must complete the secure online form. Middlebury College undergraduate students do not need to complete the form and their immunization records will be transferred internally by Middlebury’s Center for Health and Wellness. 

Please note that our secure online form requires authentication with Middlebury single sign-on credentials (i.e., username@middlebury.edu and password) and only works with Middlebury accounts that have been activated. If you have dependents and/or spouses who will be residing on a Middlebury campus with you this summer, you will need to submit this dependent/spouse form on their behalf, which also requires authentication with your Middlebury account.

Options for students who arrive non-compliant include: 

  1. Providing appropriate documentation upon arrival, verified live by staff (must be in English or accompanied by certified English translation)  
  2. Obtaining on site vaccination available at students’ expense
  3. Departing immediately at own expense with any refund being consistent with the published refund schedules

Students who choose options 1 or 2 will be granted access to their housing only after completing them. Students who choose option 3 forfeit their enrollment in their program, though they are welcome to apply again in the future.
