How to Upload Your Proof of Immunity

You can submit and/or upload your measles proof of immunity using our secure online form. This form is not for current Middlebury undergraduate students who have already provided proof of immunity to Middlebury’s Center for Health and Wellness. 

Please note that our secure online form requires authentication with Middlebury credentials (ex. username@middlebury.edu and password) and only works with Middlebury accounts that have been activated. If you have dependents and/or spouses who will be residing on a Middlebury campus with you this summer, you will need to submit this dependent/spouse form on their behalf, which also requires authentication with your Middlebury account.
