COVID-19 Vaccination Records

Dear Students,

As opportunities for vaccination arise, we have heard from some of you about providing documentation of vaccination for COVID-19. We are writing with instructions for updating your health record.

Have you received a Covid 19 vaccine?

Once you have completed your vaccine series (two doses of Pfizer and Moderna vaccines or one dose of Johnson & Johnson), please take a photo of or scan your COVID-19 vaccine record, then upload it via your student health portal. Identify your document “type” as Immunization record. Health Services staff will then enter the vaccine and dates into your immunization record.
