Below you will find details about the information and materials we will need in order to prepare for your visit.

Forms Required for Honorarium Payment

1. W9 and Contract

Guests are processed as independent contractors through Accounts Payable. Please complete the two forms below and return them to us by Wednesday, April 23.

These two forms are fillable online. To complete the document on your computer (1) download the document; (2) save the document with a new title, such as your name; (3) open the saved copy not the downloaded copy; (4) select “fill and sign” on the right of the PDF; (5) complete the document and save and return to us by email, fax, or mail.

Please be sure to list your name in the same way on both documents.

Please note that the dates on the Limited Engagement Agreement reflect the dates of the entire conference, not specifically the dates you will be visiting Bread Loaf.

The payment will be processed following the session. Payments are sent by check or direct deposit (see the direct deposit option included on the W9).

2. Online Information Form

Please submit this online form so we receive it by Wednesday, April 23.

Guest information Form

In order to complete the form, you should have the following on hand:

  • Short bio that will be distributed to participants
  • Photo that will be distributed to participants
  • Emergency contact information
  • License plate number (if driving to Bread Loaf)

Consignment Journals

Our bookstore partner, Phoenix Books will accept books, journals, and magazines to sell on consignment.  If you would like to sell your company’s publication on consignment, please read the following instructions.

In order to sell publications on consignment, please complete the Bread Loaf Guest Consignment Form by 5 p.m. EST, Wednesday, April 23. 

On the consignment form, please list your name, address, email, phone number, payment name (who to make the check out to), payment address (where to send the check), title of publication, 13 digit ISBN (if available), the retail price, and the quantity you will be bringing with you to Bread Loaf.  Please also note whether you would like any unsold copies returned to you or if you would like to donate them to the conference.  After the conference, Phoenix Books will send you a check for 80% of the sale price.  They will keep 20%.

If you do not submit the consignment form by Wednesday, April 23 we will not be able to sell your company’s publication on consignment.

You may either send the copies of your publication to us at Bread Loaf in advance of the conference or you may bring them with you to Bread Loaf.  Please note that there will be a 24-hour turn around before your publication can be sold at Bread Loaf.

Health and Safety on Campus this Summer

Required Measles Proof of Immunity 


The Vermont Department of Health is requiring that all Bread Loafers be able to provide proof of immunization from measles before the start of our summer sessions. In the time leading up to the conference participants are assigned a temporary Middlebury email and given access to a secure form for uploading their proof of immunity document. In the meantime, please note that there will be three options for fulfilling this requirement of measles immunization:

  1. obtain proof of immunization from already extant records;
  2. get a blood test that proves immunity from a doctor or clinic;
  3. get an immunization shot for measles (MMR) from a local pharmacy or doctor and provide proof of it. 

Note: Individuals born prior to January 1, 1957 are assumed to be immune to measles and do not have to provide proof of immunity. Instead of uploading a proof of immunity document via the forthcoming secure form, these individuals will provide a document showing their date of birth.  

You’ll find more details about measles and the College’s approach on Middlebury’s measles information website. (We’ll be in touch this spring with more details about your temporary Middlebury email and access to the secure form.)

Arrival and Departure

Please refer to the arrival and departure dates you have confirmed with Bread Loaf staff.

Bread Loaf Guests should check in at the Front Desk of the Bread Loaf Inn upon arrival to get your housing information.  Then, look for us in the Bread Loaf Inn Back Office (located at the back of the building). We’ll have your guest folder. The folder will contain details about the conference including a schedule of conference events. Note:  If you will arrive later than 5 p.m., please call the Bread Loaf office at 802-443-5286 to let us know.
