First Time Particpant Clearance Procedures
All students who are trying out for either an intercollegiate team or Crew and Rugby must complete the following check list before they will be eligible to participate.
1) All students are required to have a complete entrance physical examination conducted at home by a physician using the College Physical form. To be eligible to participate in athletics the physician must also sign and return the Athletic Clearance Form. Without these completed forms you will not be able to practice or play in games with the team. You can contact Parton Health Center (802) 443-5135 if you have any questions about these forms.
2) All student participants must fill out the Online NCAA Annual Health Update Form and Demographic form. Please make sure to answer all questions and fill in all Health Insurance Information. If you do not have Health Insurance please indicate that on the insurance portion of the form.
3) All first time participants are required to have a physical within 6 months of initial sports participation. If you have not had a physical within the last six months you must get one. Please use the Physical Form for intercollegiate athletes, club rugby and crew team members provided by Parton Health Center. Also provided is a list of Physicians in town that may be able to conduct the physical if you cannot get one at home. Parton Health Center does not do physicals.
4) All student participants must insure they are in compliance with the NCAA ADHD rulings, 6 month physical and sickle cell screening. Please check each page to ensure that you are in compliance with all rulings. ( Crew and Rugby DO NOT need to follow the NCAA ADHD rulings)
5) Once you have done these things you will be contacted by Sports Medicine if there is any other special documentation that they may need. (i.e. clearance from physicians for previous injuries, more documentation on pre-existing medical conditions)
