Recording release

If recording of any time, audio or video is requested, it is required for the lecturer to sign this release form.  Each box must be initialed, and the form signed at the bottom.  If the lecturer agrees to be recorded, but only for archival or classroom purposes, he/she may cross out those paragraphs with which he/she doesn't agree.
