Archive Your Work at Middlebury

Since 1800, Middlebury College students have been writing, composing, and creating scholarship and works of intellectual significance.

In 2007 Middlebury launched a digital archive of student work. Submission to the archive is contingent upon faculty approval.

As a student employee of LIS, I understand that in the course of performing my job responsibilities I will have access to information and will view information that is of a confidential nature. I understand that under no circumstances will I share this information. Furthermore, I will use this information only as is necessary to perform my job and will not use this information for any other reason. I understand that confidentiality includes, but is not limited to: the number and titles of all books a patron has on their record; contact information of patrons; internal library system notes about a patron’s borrower status or fines they owe; titles of books patrons have on hold or have recalled. I understand that a patron only has access to the above information about themselves, not about any other patrons.
I also understand that if it is determined that I have used such information for another purpose, other than that of performing my job, I will be immediately dismissed from my position and that I may be subject to disciplinary action.

Working at Circulation is a privilege. You are hired to work for a semester. If you meet the expectations outlined below, you will be invited back the following semester. Proxy Borrower Card Application

Proxy Borrowers are student research assistants, staff, or TA's designated by faculty members to check out library materials on behalf of that faculty member or department/group.

  • Faculty are responsible for all material checked out on this proxy card
  • All materials on this proxy card are subject to recall.
  • All materials checked out on this account must be returned by the end of the proxy contract.  Any materials left on this account will be automatically transferred to the faculty member's account.
  • Proxy cards may be used by many faculty or by a department, but ultimate responsibility for the items rests solely with the faculty member listed below
  • ILL requests must be made through the faculty member's Illiad account, or through a single Proxy Illiad account in the faculty's name (ILL FAQs)
  • There is no option for NExpress borrowing on a proxy account
  • Items requested via the faculty member's personal ILL or NExpress account may NOT be checked out with proxy cards
  • To view checked out items in My MIDCAT, use the primary faculty name and the barcode number located on the proxy card
  • Printing: When a student works for a department and that work requires printing, they can use a department’s generic account for printing so pages won’t be charged to their personal quota. Departments can confirm generic account information with the Technology Helpdesk. If the department does not have a generic account, the creation of one can be requested through the Technology Helpdesk.

Faculty/Staff Microsoft Campus Agreement Work at Home Acceptance Form

This acceptance form is valid for the Microsoft software products checked below, which shall be referred to collectively herein as the “Software”.Software is made available to you because Middlebury College has purchased license coverage for the Software through its Microsoft Campus Subscription Enrollment effective July 1, 2012. Middlebury College is ordering the right for you to use the Software for Work-related purposes at home under its Microsoft Campus Subscription Enrollment. You are not licensed to use the Software at home for personal purposes. You do not own the license or the CDs, rather you are authorized to use the Software and associated media pursuant to the terms and conditions of the licensing agreement during the licensed period starting 7/1/12 and ending 6/30/13. You will be required to remove the Software from your home machine immediately upon expiration of the licensed period or earlier if your employment by Middlebury College ends.

LIS Conference / Travel Approval Request

Before completing this form, you should have discussed your request intentions with your work group leader/supervisor. The final question on the form requires you to confirm that your work group leader supports this request. If he or she is unaware of the request or not in support, do not complete this form.

Once you complete this form, it will be routed directly to your Area Director based upon the Area you select in the drop-down field. After reviewing your answers and approving the request, your Area Director will forward the request to the Assistant to the Dean of LIS, who will record this request and associated estimates of expenses in our departmental files. These estimates are revised with firm expenses upon receipt of relevant invoices, receipts, etc. Your Area Director will let you know whether the request has been approved. Submittal of the form does not constitute approval.

PLEASE BE AS DETAILED AS POSSIBLE AND MAKE REASONABLE ESTIMATES OF EXPENSES. These details are important for planning purposes within LIS, and at times impact our ability to consider other requests.

General Statement: Special Collections of Middlebury College will allow qualified organizations (other museums, non-profit organizations, education institutions, governing authorities, community service groups, etc.) to borrow items for exhibition, research, education, and publication purposes. The Curator and/or Assistant Curator may deny a loan request if he/she determines that the proposed borrower would not be able to provide adequate care for the item(s) outside Middlebury College. If the borrower violates any part of this loan agreement, Special Collections of Middlebury College holds the right to terminate the agreement at any time and/or prohibit any future loans to the borrower.

1. Borrower agrees that the object(s) in this loan will remain in the condition received and will not be
repaired, restored, cleaned, or altered in any way without the permission of Special Collections of Middlebury College. The condition of the object(s) is understood to be as stated on the form.

2. All damages to the loaned object(s) at any point in this loan from wall-to-wall will be reported to Special Collections of Middlebury College immediately.

3. The object(s) may not be lent to a third party without the advance approval of Special Collections of Middlebury College.

4. The object(s) may be photographed and/or scanned only with the permission of Special Collections of Middlebury College. The borrower agrees that any photographs and/or scans made of the loaned object(s) will be used only for the purpose stated on this form and only for the number of times stated. Special Collections of Middlebury College will receive a copy of all photographs and/or scans (600dpi TIFFs - according to digital archival standards established by Special Collections of Middlebury College) for record keeping purposes. All publications of photographs and/or scans of items in this loan will bear a credit line acknowledging that the object(s) are the property of Special Collections, Middlebury College, Middlebury, Vermont.

5. Borrower will credit "[Collection] (to be supplied by staff), Special Collections, Middlebury College, Middlebury, Vermont", with ownership of the borrowed object(s) in label copy, publicity releases and in any printed matter.

6. Borrower is responsible for packing, transportation, insurance, and all other factors of transporting the loan, unless otherwise stated. The borrower is responsible for returning the loan at the time stated.

7. Special Collections of Middlebury College may require proof of insurance, may wish to have its name on the policy, and may wish to receive a copy of the insurance policy.

8. Borrower will provide evidence that the loaned object(s) are exhibited/used in an area that has climate control conditions in accordance with best practices outlined in current archival/museum standards.

9. Borrower agrees to use the loan only for the purposes stated on this form.

Special Collections Duplication, Digitization and Publication Request

Digitization

Digitization fees are charged at a rate of $10.00 per hour regardless of the material in question. The minimum fee is $10.00 with an additional $2.50 added for every quarter hour thereafter, as logged by the staff member processing the request.

Photocopying

Copying charges are: $5.00 minimum plus $.20 per page after the first 10 pages

Publication

Citation Requirement (Please note: This information will be included in the response to this form submission): Credit line should read: " ([Collection] supplied by staff) , Special Collections, Middlebury College, Middlebury, Vermont." Any material published from the Middlebury College Special Collections must be properly cited in the footnotes and/or bibliography of the publication in question.

I understand the copyright restrictions described above, and agree to abide by them as well as any applicable Policies and Procedures as required by Middlebury College Special Collections. Middlebury College does not necessarily hold the copyright to all items in Special Collections, and I understand it is my responsibility to ensure credit is given to the appropriate parties.
