Faculty/Staff Microsoft Campus Agreement Work at Home Acceptance Form

This acceptance form is valid for the Microsoft software products checked below, which shall be referred to collectively herein as the “Software”.Software is made available to you because Middlebury College has purchased license coverage for the Software through its Microsoft Campus Subscription Enrollment effective July 1, 2012. Middlebury College is ordering the right for you to use the Software for Work-related purposes at home under its Microsoft Campus Subscription Enrollment. You are not licensed to use the Software at home for personal purposes. You do not own the license or the CDs, rather you are authorized to use the Software and associated media pursuant to the terms and conditions of the licensing agreement during the licensed period starting 7/1/12 and ending 6/30/13. You will be required to remove the Software from your home machine immediately upon expiration of the licensed period or earlier if your employment by Middlebury College ends.

Work At Home rights have been ordered by Middlebury College for the following product(s) (referred to collectively herein as the “Software”):

  • Microsoft Office family products

Campus Agreement Student Users Acceptance Form

This acceptance form is valid for the Microsoft products listed below, which shall be referred to collectively herein as the "Software". Software is made available to you because Middlebury College has purchased temporary licenses for the Software through Microsoft Campus Subscription Enrollment effective July 1, 2012 through June 30, 2013. Middlebury College is extending to you the right to use the Software on a personally-owned computer or an institution-owned computer designated for your exclusive use. You do not own the license or the CDs, rather you are authorized to use the Software and associated media pursuant to the terms and conditions of the license(s) granted to Middlebury College for the term of our Campus Subscription Enrollment. You will be required to remove the Software from your personal machine immediately upon the earlier of (a) any event, with the exception of graduation, which causes you no longer to be a student of the institution or (b) expiration of the Campus Subscription Enrollment licensed period.

You are granted the right to use the following Software per the terms of the Campus Agreement Student Option:

FACULTY AND STAFF
- I will read and abide by the license agreement(s) associated with this Software.
- I understand that no technical support is provided by Microsoft in association with my work-at-home use.
- I understand the minimum specifications to run the Software are listed at http://www.microsoft.com/products.
- I will remove the Software from my home machine immediately upon the earlier of (a) expiration of the licensed period specified above or (b) the end of my employment by (name of institution).
- I understand that I am not licensed to use the Software for personal purposes.

STUDENTS

- I will read and abide by the license(s) associated with this Software.
- I understand that no technical support is provided by (enter name of institution).
- I understand the minimum specifications to run the Software as listed.
- I understand that I will be required to remove the Software from my personal machine immediately upon the earlier of (a) any event, with the exception of graduation, which causes me to no longer to be a student of the institution or (b) expiration of the Campus Subscription Enrollment licensed period.
- I understand that if (enter name of institution) does not renew the Student Option under the Campus Subscription Enrollment, then I must delete or remove the Software licensed to (name of institution) under the Campus Agreement from my computer at the time the enrollment expires or is terminated.
- I understand that if I graduate from (name of institution) during the Campus Subscription Enrollment licensed period, the Software license will convert to a perpetual license (meaning I have full ownership of the Software).
- I understand that it is my responsibility to contact the institution to obtain the Student License Confirmation, which is proof of Software ownership, prior to graduation from the institution.
