Health Information and Forms for First-year and Transfer Students
The staff of Parton Center for Health and Wellness joins in welcoming you to Middlebury College! Parton offers integrated care for students through our Health Service, Counseling, and Sports Medicine departments. You can learn about our services and available resources using the links on the left side our our web pages, and through many other available links throughout the site.
As you prepare for the 2013-2014 academic year, we want to fill you in on a few important Middlebury College health requirements. Please share and discuss this information with your parents or guardians and your health care provider prior to arriving on campus.

Student Health Form
Middlebury College requires a completed Middlebury College Student Health Form, which includes your Personal Health History and a physical examination, review of immunizations and TB screening by a health care provider. If you've had a physical exam since September 1, 2012, and are not planning to play an intercollegiate sport or club Rugby or Crew, your doctor may not need to perform another one. However, the Student Health Form must still be completed and signed by your health care provider.
The 2013-2014 Middlebury College Student Health Form is due by July 1, 2013 for September matriculation and October 1, 2013 for February 2014 matriculation.
If you have a significant medical or mental health history, send copies of:
- Major test results and consultant reports (i.e. echocardiograms, MRIs, psychiatric evaluations)
- Surgical reports
- Treatment plans (including Asthma Action Plan, psychiatric treatment plan, relapse action plan)
If you choose not to be immunized based on medical, philosophic or religious reasons you must complete the Immunization Exemption Form and submit it to Health Services.
Health Insurance
Middlebury College requires that all students have adequate health insurance. Review your health insurance benefits with your parent or guardian. Save yourself and your parents the surprise of uncovered medical costs by being familiar with your insurance coverage.
If you are covered by private health insurance, copy both sides of your insurance card and enclose it with your completed Student Health Form.
Mail In Your Forms
When all the paperwork is complete, your physical is done and your health care provider has signed everything, make a copy for your records and then return it all together prior to the July 1st deadline to:
