Scheduled appointments with Sports Medicine are only for varsity athletes, and students on a active rugby or crew roster.
Appointments are scheduled from 10 a.m.–2:30 p.m., Monday through Thursday, and each appointment slot is 30 minutes. After 3 p.m., our staff’s time is reserved for in-season athletes.

Please note: Athletes must have all their completed Sports Medicine forms in the Student Health Portal before they are eligible for services.
First-year and first-time athletes on a roster are allowed to make appointments with Sports Medicine one month before the start of their season. Winter sports athletes are eligible after October 1, and spring sports athletes are eligible after January 15.
Concussion testing will be done upon your arrival to Middlebury and scheduled with your athletic trainer.
Middlebury College contracts with UVM Health Network Porter Rehabilitative and Orthopedic Services (PROS) to provide physical therapy treatment to our existing student athletes. Any incoming first year or transfer student that comes to Middlebury needing physical therapy is ineligible for services and is directed to providers in the community. Our Administrative Associate sets up the initial appointment for the student within three business days after completing all the required paperwork. The completed paperwork is sent to UVMHN PROS for insurance and eligibility verification. Any cancelations are done through the physical therapist or the administrative associate. Any billing inquiries are directed to UVMHN PROS as they are a separate entity and do bill for their services.
UVMHN PROS primarily provides services for the following:
• Post-surgical physical therapy
• Out of season physical therapy
• Long term rehab
Physical therapy eligibility:
• Returning athletes post-surgical with a PT prescription
• Returning out of season athletes referred by AT or with a PT prescription
• Athletes who have suffered a season ending injury
Not eligible for physical therapy:
• Anyone not on a current roster
• First year athletes who will not compete in their first season
• First year winter/spring sport athletes that are not eligible for access to Sports Medicine until one month prior to when their season starts.
