The National Institutes of Health requires Middlebury College to secure and retain each PI’s signature as part of the internal proposal endorsement procedure for all grant applications, including competitive renewals. This signature is collected on a separate form attached to the endorsement form (formerly known as the "blue sheet").
This signature certifies that:
- the information in the application is true, complete, and accurate to the best of the PI’s knowledge
- the PI understands that false statements or claims may result in criminal, civil, or administrative penalties
- the PI accepts responsibility for the scientific conduct of the project and will provide required progress reports if a grant is awarded
- The PI agrees to abide by the terms of the Bayh-Dole Act and the NIH Grants Policy Statement as they relate to intellectual property rights
