Major/Adviser Declaration or Change Form
Use this form to declare a major in Spanish. The form must be signed by the chair of the department.
Minor Declaration Form
You can declare a minor in Portuguese or Spanish (or both!) with this form. Any professor in the department can sign this form.
Tutor Form
