Below are the forms related to applying for Optional Practical Training (OPT) or STEM OPT followed by the forms related to reporting requirements while on periods of OPT or STEM OPT.  These forms and resources are the same for Middlebury College and Middlebury Institute students.

Reporting forms:

• Address Update
• New Employment Update Form
• End of Employment Form
• STEM OPT Verification Form
• Updated I-20 Request Form (typically used for travel signature requests)
• SEVIS transfer out form (for use if transferring your SEVIS record from Middlebury to another U.S. institution)
