Incident Reporting
A safety incident (accident, fire, spill, animal bite, etc.) is an event that results in an injury or illness to an individual, damage to equipment/building, or a “near miss”. It is important to follow proper reporting procedures after a safety incident.
Complete the Middlebury Sciences Incident Reporting Form for all safety incidents.
If a Safety Incident Involved an Injury
- Faculty and staff must report all injuries to manager/supervisor immediately. Report to Human Resources within 24 hours of the incident.
- Students must report all injuries to your supervisor or lab instructor immediately.
In the event of accident or medical emergency, call the Department of Public Safety at x5911 (802-443-5911) or dial 911.
