September Pre-Arrival Checklist

Please keep an eye on this checklist.  We will continue to add information throughout the summer.

Middlebury College E-Mail Account

• Step One: go.middlebury.edu/activate to set up your password.  You need your college ID number and your BannerWeb PIN to run this program.
• Step Two: go.middlebury.edu/webmail to login to your email account
• SUBMIT: right now! 

Self-Service Banner Forms

• Log into go.middlebury.edu/ssb and fill in required information
• SUBMIT: the first time you log into Self-Service Banner you will be asked to provide some personal information; please take the time to answer these questions thoroughly when you log in

Trips Registration Form

• Submit information via the online form
• SUBMIT: by June 1

First-Year Seminar Registration (for first-year students only)

• Specific registration information will be included in a June mailing
• Register via Self Service Banner
• SUBMIT: anytime between June 13 at 9:00 a.m. through June 20 at 5:00 p.m.

Housing Contract and Roommate Questionnaire

• Submit the contract by logging into go.middlebury.edu/ssb and filling in the required information
• Submit the questionnaire by logging into Self-Service Banner and clicking on personal information; use the Housing Information link once all pre-forms have been submitted
• SUBMIT: Questionnaire by June 21 for first years and August 1 for transfer and exchange students

Health Forms

• Step One: go to go.middlebury.edu/healthforms
• Step Two: print and complete the forms; return them via mail
• SUBMIT: by July 1

ID Photo Submission

• Submit online at go.middlebury.edu/idphotos
• SUBMIT: between July 9 and August 23

Special Needs Identification Form

• Submit information via the online form or contact Student Accessibility for an alternative format
• SUBMIT: by July 15

Online Placement Exams

• The placement exam chart will be posted on the website in June
• SUBMIT: sometime between August 1 and September 3; ideally the exams should be taken in advance of your arrival on campus

Insurance Waiver/Enrollment Forms

• Log onto www.gallagherkoster.com/Middlebury and carefully read the instructions on the site
• SUBMIT: by August 1

Parking Permit Registration

• Go to Banner Web to pre-register your vehicle
• Go to the Public Safety Store Front to pay for your permit via credit card
• SUBMIT: between August 1 and August 26

MyStudentBody Modules on Alcohol, Drugs and Sexual Violence

• Watch for an e-mail sent to your Middlebury account in late July
• SUBMIT: between August 5 and August 30

Fall Tuition Payment

• Notification will be sent via e-mail to your Middlebury account; access via QuickPay
• SUBMIT: by August 15

Insurance Waiver/Enrollment Forms

• Log onto www.gallagherkoster.com/Middlebury and carefully read the instructions on the site
• SUBMIT: by August 15
