This is only for first year students; transfer and exchange students do not enroll in First-Year Seminars and will complete course registration when they arrive on campus. Please note that registration for First-Year Seminars starts at NOON on June 11 and stays open until June 18 at 5pm for September admits. Febs will register for First-Year Seminars in January.

Student Health Form (Including Personal Health History, Physical Exam, and Immunization Record)

All students must have health insurance; you must take action and choose to either enroll or opt out of the Student Health Insurance Plan. 
New this year, we will be requiring the same form for all entering students, including athletes. If there is any possibility that you will participate in an intercollegiate sport or club rugby or crew, the NCAA requires that your physical exam must be completed within 6 months of the start of your athletic season.

Authorized Payer Information

Middlebury is currently transitioning to a new e-billing system, which will be live in early July. At this time, you will be able to update your Authorized Payer Information.

Ebola Virus Notification

Review Parton Health Center’s web page for the most up-to-date information regarding Ebola Virus Disease and related travel restrictions as well as other guidance that might impact your studies at Middlebury.

Health and Wellness Courses

Prior to arrival on campus, all incoming students are REQUIRED to complete two health and wellness courses that address alcohol, drugs, relationships, sexual violence, bystander intervention, and harassment.  If you are an international student with limited access to the Internet or a student who has yet to complete the courses, you will be required to complete the courses shortly after arriving on campus.

Tuition Payment

Fall 2015 billing notifications will be forwarded electronically in early July. Fall semester is due August 15 and the spring semester is due December 15. Middlebury will be transitioning to a new e-billing system. The system will be live in early July. More information will be sent out to students and families in early June. This e-billing system will also incorporate a monthly payment plan.

September Deadlines

Display/Update Preferred First Name and Pronoun

Any student can choose to be addressed by the first name that they prefer regardless of whether or not they have legally changed their name. Students can also indicate the gender pronoun they choose to identify with and would prefer others use when talking to or about them.

All students must have health insurance; you must take action and choose to either enroll or opt out of the Student Health Insurance Plan. 
To enroll or waive the Student Health Insurance Plan for the 2015-2016 policy year:
1.       Log onto:
2.       Click on “Student Waive/Enroll”.
3.       Enter your Middlebury assigned student email ( as your User ID and your student ID as your Password.
4.       Select the Blue “I want to Waive/Enroll” button.
5.       Please fill out the decision form and select either “I want to Enroll” or “I wish to Waive” the Student Health Insurance for the 2015-2016 policy year. Upon completing your online form you will be asked to review the information provided and click ‘Submit’ to complete the process. Immediately upon submitting your Online Decision Form, you will receive a reference number.
