COVID-19 Vaccination Upload Process
Language Schools faculty, students, and staff:
Middlebury Language Schools is working with Sentry MD to collect required COVID-19 vaccination records for this summer.
Proof of your up-to-date COVID-19 vaccination must be uploaded by June 14. Up-to-date means you have received one booster dose after a primary series. You will need to use your middlebury.edu email address and have your vaccination documents to complete the uploading process. Please follow the steps listed below.
- Step 1: Obtain COVID-19 vaccination record(s). (Primary series and booster)
- Step 2: Sign the authorization of release.
- Step 3: Submit both documents (Step 1 & Step 2) to the Sentry MD Portal.
Please note, returning faculty/staff/students and Middlebury College undergraduates must also complete this process through Sentry MD. To request a religious or medical exemption, please complete the Exemption Form and upload the form via the Sentry MD Portal.
