OPT Application Checklist

Follow these steps to prepare your OPT application.

Request a new I-20 from the international student advisor. Fill out the online OPT application to request the I-20.
Begin gathering your OPT application materials (see section below).
International student advisor will reach out to schedule a meeting to process the OPT request. Please bring all OPT application materials to the meeting.

Please provide the following:

Completed and signed Form I-765. We recommend that you fill out the form online before printing.
Two photos in a photo holder or envelope. Photos must be a full-color frontal pose on a white background (passport-style photo). Write your name and SEVIS number gently on the back of each photo in pencil or felt pen.
One copy of the new SEVIS I-20 with the OPT recommendation.
Copies of all former I-20s you were issued since your arrival in F-1 status.
One copy of the I-94 card, found here.
One copy of the F-1 visa.
One copy of the passport biography page.
Check or money order—application fee can be found in the Form I-765 instructions.
OPTIONAL: Complete the G-1145 E-Notification of Application/Petition Acceptance form.

Send your application by certified mail with a return receipt or by express mail to the appropriate lockbox.
The OPT application must be received by USCIS within 30 days of OPT recommendation being added to your SEVIS record.
