Use the checklist to make sure you have completed your application correctly.

  1. Complete the DS-2019 Application.

  2. Photocopy of informational page(s) of your passport showing your name, date of birth, and passport expiration date. Most passports have this on one page, but in some cases you may need to submit multiple photocopied pages.

  3. Sponsor letter: If you are going to receive financial support from a third party sponsor, please submit a financial guarantee letter with the following information:

    • Date document is provided

    • Name of your sponsor

    • Duration of financial sponsorship

    • Expenses covered under financial sponsorship (tuition, health insurance, cost of living, etc.)

    • Letter must be signed by sponsor and should be on letterhead from sponsoring organization or government 

  4. Bank letter/statement that is in English, is issued no earlier than six months prior to your program start date, and contains the following information:

    • Bank name, preferably on the financial institution’s letterhead and/or stamped by bank personnel

    • Name of the account holder

    • Amount of funds available

    • Currency of funds, preferably in USD

      • If your bank cannot issue the statement with currency conversion, the document must clearly state the currency of funds listed. If your financial institution is unable to issue a statement in English, a certified translated copy can be submitted with the original document.

    • Type of account

      • Funds need to be immediately available to the student, such as in a checking/savings account.

      • Funds that are invested (mutual funds or tied-up assets) cannot be used to show financial support.

  5. Dependent information for any dependent(s) that will accompany you, including a copy of their passport information page and show of additional financial support. A dependent is defined as a spouse or child.
