Diploma Reprint Requests: Middlebury will issue a replacement diploma, only if one was lost or damaged. We do not supply multiple copies. To request a replacement diploma, create an account with Parchment and order a replacement diploma through their platform.

After filling out the PDF form, save with changes. If signatures are required, please obtain these before turning in your form. You may attach email approval in place of a signature.
The Petition for Waiver of Academic Regulations may require one signature or all signatures, depending upon the request. Please contact the Records Office with any questions.
